We at Shipping Solutions like to promote the fact that our export documents can be printed with a regular printer on plain paper. There is, however, one exception: the U.S. Certificate of Origin for Exports to Israel.
The Israeli government requires that the Certificate of Origin be printed on their official green form. We are not allowed to create a similar form in Shipping Solutions, but we do have a “data only” form that will print everything in the correct spot on the official form.
You can purchase the Israeli Certificate of Origin from the U.S. Commercial Service Minnesota office at (612) 348-1637.

To prepare the form in Shipping Solutions fill out the data shown below, which is listed by the boxes on the U.S. Certificate of Origin for Exports to Israel and includes the screen in Shipping Solutions where the data item is entered. Then, print the data on an official form.
Box 1 – EZ Start Contacts screen, Exporter section, Company, Address 1, Address 2, City, State, Postal Code, and Country fields.
Box 2 – EZ Start Contacts screen, Ultimate Consignee section, Company, Address 1, Address 2, City, State, Postal Code, and Country fields.
Reference No. box – NAFTA/Other Free Trade Agreements screen, Israeli Certificate of Origin section, COO Reference Number field. This field should indicate sequentially during each calendar year the number of the shipments sent to Israel. For example, the first shipment sent in 2015 should have the reference number of 2015-1, the second shipment should have the reference number 2015-2, etc.

Box 3 – NAFTA/Other Free Trade Agreements screen, Israeli Certificate of Origin section, Means of Transport field. Enter the name of the ship or airline and identifying number (if applicable) along with the route to be taken.
Box 4 – Leave blank.
Box 5 – This is an auto-generated line number.
Box 6 – Product Detail screen, Bill of Lading section, Marks & Numbers field.
Box 7 – Product Detail screen, Bill of Lading section, Number of Packages and Type of Packages fields and the Product Description field at the top of the screen.
Box 8 – Product Detail screen, NAFTA/Other FTAs section, Israeli COO Criterion field. If the goods are wholly grown, produced or manufactured in the U.S. enter the letter P. If the goods are not wholly grown, produced or manufactured in the U.S. enter the letter Y and the percentage of value added in the U.S. and/or Israel. For example, if half the value was added in the U.S. enter Y=50%.
Box 9 – Product Detail screen, AES/EEI section, Gross Weight field.
Box 10 – Invoices-Proforma/Commercial screen, Commercial Invoice Date and Commercial Invoice Number fields.
Box 11 - If the Exporter is not the Producer of the goods, the certificate needs to be signed by your local Chamber of Commerce. If the Exporter is the Producer, simply sign below the "Exporter as Producer" statement.
Box 12 - The Exporter must sign the certificate and have it notarized.
